Job Description

Responsible for the programmatic development, planning, organization and management of the functions that focus on care facilitation and movement of patients through the hospital continuum. 

This includes, but is not limited to: Case Management, Social Work, Utilization Management and post-acute services coordination. Develops the Hospital Case Management program to enhance the distinction in staff roles related to progression of care, physician.

Permanent and/or Interim

Minimum Required: 

• Montana Registered Nurse license/Compact RN License 
• Minimum of 5 years case management in healthcare setting
• Proven communication and customer relation skills

NICE TO HAVES :

• Masters degree preferred
• 3-5 years supervisory role
• Formal case management training
