JOB DESCRIPTION
The Director, Community Partnership Program, is a key leadership position in the Population Health Services Division at the Hospital. The Population Health Services Division serves as the central hub for programs that serve patients at the hospital and across partner provider organizations across the state of Massachusetts as part of our Medicaid ACO program.
The Director will serve as their strategic and operational leader for the MassHealth (MH) Community Partners (CP) program. Community Partners are community-based care organizations that partner with ACOs to provide care coordination / care management services to Medicaid members. LTSS Community Partners focus primarily on members with LTSS / SDOH needs, and Behavioral Health Community Partners focus primarily on members with BH needs. Across the state of MA, there are multiple LTSS and BH CPs that currently partner with the hospital Medicaid ACOs. The Director will oversee all aspects of the the hospital Community Partner program: developing an overall strategic vision for the hospital’s role in the program, CP relationships/partnerships and contracting, CP analytics/measurement, operational oversight for internally-managed aspects of the program, and program advocacy. In addition, the Director of the hospital’s LTSS Community Partner, Boston Allied Partners, will report in to this leader. The Director will partner closely with the Population Health team, the hospital Complex Care Management team, and other key clinical and administrative leaders working on the Medicaid ACO program.

ESSENTIAL RESPONSIBILITIES / DUTIES:
Design a coordinated system-wide CP strategy: ACO provider organizations, the hospital, and community organizations currently provide a broad range of care coordination programs. They represent a diverse set of programs – who they serve, where they operate, how they are paid, how success is measured. The Director will work closely with these program leaders and other the hospital system leaders to define the role and goals of the CP programs in this broader care coordination landscape. They will develop a unified vision and program road map for the CP program that optimizes clinical outcomes, financial viability, and patient satisfaction, as well as meeting state and other regulatory body requirements.
CP Contracting and management: Oversee contracting and contract management strategy for all CPs. The hospital plans to design a preferred network for coordination services with clear accountability for clinical and financial outcomes.
State advocacy and partnership: EOHHS defines many parameters for care coordination and management. The Director will represent the hospital interests and perspectives in state relationships, and work collaboratively with EOHHS to define policy and programs that meet shared goals for care coordination.
Data and Analytics: Partner with Clinical and Financial Analytics teams to define opportunities to optimize CP program design performance; robust data analytics skills required
Translation of data into program actions: Ensure ACO programs are data-driven; translate data-driven insights into clear priorities and interventions.
Program Operations: Work in close partnership with CC and CM program leaders to optimize CP program operations for Boston Allied Partners (BAP).
IT program optimization: Partner with IT leadership to optimize IT supports and integration for CP partners.
Local site engagement and communication: Partner directly with ACO site leads on local CP-Primary Care integration strategies, including clinical partnerships, communications, workflows, and reporting.
