Social Work - Case Manager - Las Cruces, NEW MEXICO - USA at Crescentia Global Talent Solutions

Manages patient's transition through the healthcare system and transfers accountability to the appropriate person or agency upon discharge. Performs needs assessment upon admission, concurrent and retrospectively.  

Provides necessary information to 3rd party payers, physicians, staff, patients and families. 

 Provides services to patients throughout the lifespan including all age groups.  Culturally sensitive to the needs of all patients.  Knowledge of local community resources is helpful.  

Experienced with complex discharge planning.   Accountable for accurate resource utilization and facilitation of improvement in quality care for assigned caseload.

Must-Haves:

• Graduate of a Social Work BSW or MSW program.
• Must possess familiarity with case management issues and discharge planning.
• Must possess a current New Mexico license as a Social Worker.

Nice to Have:

• Case Management Certification preferred
• Familiarity with Comprehensive Addiction and Recovery Act (CARA) and associated care plans
• Minimum one year experience
