The Active Wellbeing Programme:
The Active Wellbeing programme is a specialist mental health exercise referral programme. This programme is designed to engage and promote health and wellbeing to people diagnosed with a severe mental illness (SMI). The role will be working across South West London with a primary focus on Kingston.
This outreach and engagement role will see the successful applicant working across multiple BME (Black and Minority Ethnic) communities with consideration, experience, and sensitivity to the needs of a wide range of minority ethnic groups and cultures who have a diagnosis of a long and enduring mental health condition(s).
Reports to: Senior Mental Health Programme Lead
Based: South West London
Salary: £30,954
Contract: 12 months fixed term contract

Main Duties and Responsibilities:
- This role is responsible for conducting one-to-one assessments with people with Severe Mental Illnesses and enrol service users onto the Active Wellbeing Programme with a primary focus on Wandsworth and possible other South West London Boroughs
- Identify and build key relationships with BME communities and those with an SMI that may not actively be accessing healthcare and physical activity
- Identify and build key relationships with other community services that may benefit those participates on the active wellbeing programme
- Responsible for conducting one-to-one assessments with people with Severe Mental Illnesses and enrol service users onto the Active Wellbeing Programme with a primary focus on Wandsworth and possible other South West London Boroughs
- Monitor and report key outcomes and impact of the services for each individual.
- Involved in the development and delivery of programme workshops and events for both service users and partners
- Promote the service positively through different social media sites and case studies.
The successful candidate will have:
- Experience in working with people with long and enduring mental health conditions and/or severe mental illness in the community or clinical settings
- Knowledge & understanding of BME communities and the associated cultural, religious and wellbeing considerations for accessibility to health care and physical activity
- Knowledge and Understanding of how physical activity can benefit people who are affected by Severe Mental Illness (SMI)
- Knowledge & awareness of relevant national, regional and local policies and initiatives within health, physical activity & public health
- Knowledge of Wandsworth, Richmond & Kingston and the community organisations that operate in the borough (desirable)
- Experience in hosting outreach events, partner meetings and service users events
- Experience in reporting safeguarding issues and concerns with regards to vulnerable adults when necessary
- Establishing and sustaining positive relationships with stakeholders and partners
- Flexibility; willingness to work at multiple locations, and work hours required by post including early mornings, evenings, weekends and bank/public holidays
