About Success4Life

Success4Life is a multi-state human services agency providing community-based, skill-focused supports through Wisconsin’s Comprehensive Community Services (CCS) program. We partner with counties to deliver person-centered services that help individuals build independence, daily living skills, and community stability.

We are expanding services in Rock County, Wisconsin and are seeking local CCS Providers to join our provider network.

What is CCS?

Comprehensive Community Services (CCS) is a Wisconsin Medicaid program that supports adults and youth through skill-building, goal-oriented, and community-based services. Services are individualized and based on each participant’s service plan.

Position Overview

CCS Providers work directly with adults and/or youth enrolled in the Rock County CCS program. Services are delivered in the community and focus on functional skill development and practical supports identified in the consumer’s service plan.

This role offers flexibility, autonomy, and meaningful work, while allowing providers to maintain independent practice.

Responsibilities

• Provide CCS services aligned with approved service plans
• Support skill development in areas such as:
• Daily living skills
• Organization and time management
• Employment-related skill development
• Community integration and independence skills
• Maintain timely and accurate documentation in accordance with CCS and county requirements
• Communicate as needed with service facilitators and county partners
• Maintain professional boundaries and ethical standards

Qualifications

• Eligible to provide services under Wisconsin CCS (required)
• Background in human services, social work, education, psychology, peer support, or a related field
• Experience working with adults and/or youth with mental health or substance use challenges preferred
• Strong communication and organizational skills
• Ability to work independently in community settings
• Reliable transportation and ability to travel locally within Rock County

Training

Required CCS and compliance training is provided and completed asynchronously on your own schedule prior to delivering services.
