FAMILY NURSE PRACTITIONER FNP JOB DESCRIPTION

• Well established Community Health Center
• Work with a team of providers and support staff.
• This provider will be treating all age groups.
• Manage a patient panel and able to see on average 20 patients per shift.
• 4 day work week with no weekends or holidays.

FAMILY NURSE PRACTITIONER FNP JOB RESPONSIBILITIES

• Diagnose and treat complex and acute primary care medical conditions
• Perform comprehensive evaluations including histories and physicals
• Order diagnostic laboratory and radiology tests
• Develop and implement patient management plan of care
• Prescribe medications as appropriate
• Perform timely documentation of medical records

FAMILY NURSE PRACTITIONER FNP JOB REQUIREMENTS

• Graduate of accredited nurse practitioner program
• Minimum of 3 years of similar experience
• Experience managing a patient panel
• Have an interest or a history working in a CHC setting
