Overview 

 

As a HIU Health and Wellbeing Coach you will help High Intensity users develop the knowledge, skills, and confidence to manage their own health and wellbeing.  The HIU Health and Wellbeing Coach will use health coaching, motivational interviewing and behaviour change skills to empower people to make positive lifestyle changes. The role will take a holistic approach that helps increases people’s ability to access community support and self-management education.  

 

The successful candidate will join our Enable Mental Health Team which has experience of delivering community support across London. The Health and Wellbeing Coach will work alongside Clinical Leads and other health professionals to provide a joined-up approach and maximise patient experience.

This role is fixed term until 31 March 2023, with the possibility of extension 

 

Job Purpose 

The post holder will act as high intensity user lead working with high intensity users (HIUs). Through direct contact with the client group, seek to appropriately reduce the number of A&E attendances, non- elective admissions and potentially 999 calls. This will deliver measurable improved outcomes for the benefit of clients, staff and the public. 

The main focus includes unmet social needs that present mental health episodes to A&E. They may not be accessing scheduled services and therefore rely heavily on unscheduled services. Personalisation is pivotal to this role, actively listening and working together to underpin changes in client behaviour rather than resorting to punitive measures.

Main Duties and Responsibilities 

Person-centred care and support  

  • Deliver effective health coaching sessions using a holistic and person-centred approach that helps people focus on what matters to them.  
  • Empower people to take control of their own wellbeing by building their confidence, motivating them to set goals and creating personalised action plans. 
  • Support people to overcome their personal barriers to make positive lifestyle changes. 
  • Provide ongoing motivational support and follow up to ensure long term behaviour changes.  
  • Support people to access to appropriate community support and self management education. 
  • Treat everyone with dignity, and respect different cultures, religions, ways of life and other personal characteristics at all times.  

Expected Outcomes:  

  • Impact positively on reducing the amount of high intensity use emerging to replace those already managed. 
  • To support clients to flourish through sustaining job opportunities, reconnecting with families, improving well-being etc. 
  • A new culture of health coaching as a medium to deliver sustainable change. 
  • It is recognised that the latter two points of expected outcomes are more difficult to measure but they are essential outcomes if a culture change is to occur to lower the stigma associated with this cohort. 
  • The post holder is responsible for creating an innovative way of supporting high intensity use of A&E. They will facilitate discussions and advise colleagues as to how best practice might be adopted for future development of the service and oversee their delivery.  
  • Lead in removing potential barriers and stigma associated with HIU to promote equality, diversity and safeguarding service-wide.  
  • The clinical lead will act as an advocate for the client, guiding them through the complex journey and multi-faceted approach to encourage appropriate use of scheduled and unscheduled care services. 
  • A further element of the role would be coordination, sharing and learning of the work with community based staff to promote safe practice and sustainability.  
  • The post holder must be proficient in creating and maintaining Community Care Plans in conjunction with the client to provide bespoke care in a preferred setting. 
  • The post is responsible for providing professional expertise to the outcome of the business processes for Clinical Commissioning Group’s including report writing, presenting to the governing bodies. 

 

Key Working Relationships  

 

  • The post holder will be required to communicate and provide highly complex information to a wide range of internal and external stakeholders.  
  • The post holder should have robust and active links with the A&E and unscheduled care teams with whom they will be working in partnership or be competent in connecting well with other partners.  
  • Commit to working and engaging constructively with internal and external stakeholders on a range of highly sensitive issues.  
  • Nurture key relationships and maintain networks internally and externally, including national networks. This includes building relationships with wide range of supporting organisations, 3rd sector groups that may provide services of benefit to the HIU cohort. 

 

Communication and Collaboration  

  • Work alongside other Primary Care professionals to provide joined-up personalised care and develop the service model.  
  • Build excellent relationships with primary care teams and other key stakeholders.  
  • Build knowledge of community support and develop partnerships to ensure signposting is safe and effective.   
  • Act as a champion of personalised care and health coaching, increasing awareness and promoting its benefits to primary care staff and other audiences.  

 

Service Quality and Development  

  • Collect and record outcome, monitoring and impact data using required systems, contributing to the evaluation of the service.  
  • Manage own case load to ensure clients receive appropriate levels of support and expectations are met.  
  • Gather client feedback to evaluate the quality of the service and to inform service changes and developments.   
  • Keep up to date with key changes in the health and social care sector in order to shape and develop the service, including those relating to good practice and policy.  
  • Lead by example by demonstrating excellent customer service, delivery skills, organisation and professionalism. 
  • Work with your line manager to identify your own support needs and undertake continual personal and professional development.

  • To be fully aware of the principles of safeguarding as they apply to children and vulnerable adults in relation to your work role. Always follow safeguarding procedures and ensure that your line manager is kept fully informed of any safeguarding concerns. 
  • To carry out any other reasonable duties and responsibilities that contribute to the overall function of the team, appropriate with the grading of the post.  
  • To apply a flexible approach to working in different environments including from home, GP practices, healthcare settings, community venues and Enable offices.  
  • To travel inside or outside the borough when required.
