Job Description: We are seeking a dedicated and experienced Registered Nurse (RN) to join our hospital’s Case Management team. The RN Case Manager will be responsible for coordinating and overseeing patient care throughout their hospital stay, ensuring efficient, high-quality care while promoting positive outcomes. This role requires strong clinical knowledge, excellent communication skills, and the ability to collaborate with healthcare teams to optimize patient care.

Responsibilities:

• Assess patient needs and develop comprehensive care plans in collaboration with physicians, social workers, and other healthcare professionals.
• Coordinate patient care across multiple departments, ensuring appropriate resources and interventions are provided.
• Monitor patient progress and adjust care plans as necessary.
• Educate patients and families on treatment options, discharge planning, and community resources.
• Ensure timely discharge by arranging post-hospital care and follow-up appointments.
• Document patient progress, interventions, and discharge plans accurately in the electronic health record.
• Advocate for patients and facilitate the delivery of cost-effective care while maintaining quality standards.
