Job Description: We are seeking a compassionate and experienced Registered Nurse (RN) to join our Home Health team. The RN will provide high-quality, patient-centered care in patients' homes, assisting with recovery, chronic disease management, and maintaining overall health. This role requires excellent assessment skills, the ability to work independently, and a commitment to improving patient outcomes in a home setting.

Responsibilities:

• Conduct comprehensive patient assessments, including physical, emotional, and social well-being.
• Develop and implement individualized care plans in collaboration with the patient, family, and healthcare team.
• Administer medications, treatments, and procedures as prescribed, including wound care and IV therapy.
• Monitor patient progress, vital signs, and respond to any changes in condition.
• Provide education to patients and families on disease management, medications, and self-care techniques.
• Coordinate care with physicians, therapists, and other healthcare professionals.
• Document all patient interactions, treatments, and progress in the electronic health record.
