Job Title: Case Manager at Woodlake Addiction Recovery Center
Woodlake Addiction Recovery Center is seeking a highly motivated and compassionate individual to join our team as a Case Manager. As a premier addiction recovery center, we are dedicated to providing our clients with the highest quality of care and support in their journey to overcome addiction. Key Responsibilities:
• Conduct comprehensive assessments of clients' needs and develop individualized treatment plans
• Collaborate with clinical staff to coordinate treatment and ensure the implementation of the treatment plan
• Provide case management services, including referrals to external resources and follow-up appointments
• Monitor client progress and provide guidance and support throughout their recovery journey
• Communicate and collaborate with clients' families and loved ones to ensure a holistic approach to treatment
• Maintain accurate and confidential records of clients' progress and treatment plans
• Participate in team meetings and provide updates on clients' progress and any challenges or concerns
• Keep up-to-date with industry trends and best practices to continuously improve the quality of care provided
Qualifications:
• Bachelor's or Master's degree in social work, psychology, counseling, or related field
• Minimum of 2 years of experience in case management or related field
• Knowledge of addictions, mental health, and trauma-informed care
• Strong communication and interpersonal skills
• Ability to work collaboratively as part of a team
• Experience with electronic medical records

Salary: $86,000-107,000 per year
